1. Competition details form part of these terms and conditions
  2. Entry is open to employees or business owners of Lancashire businesses, except employees of Active Lancashire, statutory organisations and any delivery partner of the Business Health Matters programme.
  3. The person entering on behalf of a Lancashire business must have the permission of the organisation they are employed by to enter the competition.
  4. The competition will run from Thursday 24th June 2021 until midnight on Tuesday 31st August 2021.
  5. Entrants must be 18 or over and must be willing to provide full name and contact details, including the name of their business. Use of a false name or address will result in disqualification.
  6. The newsletter registration form linked HERE must be completed in full to be considered as a valid entry and can only be entered electronically. Entrants must still be subscribed to the newsletter by 31st August 2021 to be entered into the prize draw.
  7. On sign up to the newsletter, you will receive confirmation that you have been added to the mailing list. Please ensure you have received this and check junk filters to ensure this has been received.
  8. Only one entry per business will be permitted.
  9. All entries must be made directly by the person entering the competition and entries made online using methods generated by a script, macro or the use of automated devices will be void.
  10. No responsibility can be accepted for entries lost, delayed or corrupted, or due to computer error in transit.
  11. The winner will receive up to 20 health checks for their businesses. Please note, subsequent optional wellbeing packages following the health checks are not included in the prize and are chargeable if chosen by the business.
  12. The prize is as stated, is not transferable to another individual or business and no cash or other alternatives will be offered.
  13. The prize of up to 20 health checks can be claimed within 12 months of the competition end date.
  14. The winner will be contacted within 28 days of the competition ending.
  15. The promoters reserve the right to amend or alter the terms of competitions and reject entries from entrants not entering into the spirit of the competition.
  16. In the event of the prize being unavailable, the promoter reserves the right to offer an alternative prize of equal or greater value.
  17. The winner agrees on behalf of their business to the prospective use of their name, photograph and disclosure of county of residence and will co-operate with any other reasonable requests by Business Health Matters relating to any post-winning publicity. The winner can opt out by writing to Business Health Matters via [email protected] or via the postal address below.
  18. The winner will be drawn at random from all qualifying entries received by the closing date stated within the promotional material.
  19. Reasonable efforts will be made to contact the winner. If the winner cannot be contacted, or is unable to comply with these terms and conditions, the promoter reserves the right to offer the prize to the next eligible entrant drawn at random.
  20. Failure to respond and/or failure to meet the eligibility requirements may result in forfeiture of the prize.
  21. Competitions may be modified or withdrawn at any time.
  22. The provider and promoter of the competition is Business Health Matters c/o Active Lancashire, Office 3&4, Farington House, Lancashire Business Park, Centurion Way, Leyland PR26 6TW.
  23. In the event of a discrepancy between these standard terms and conditions and the details in the promotional material (or any other terms and conditions provided/referred to at the time of entry), the details of the promotional material (and any other terms and conditions provided/referred to at the time of entry) shall prevail.
  24. Any personal information supplied will be used to sign up to the Business Health Matters newsletter and administration of the prize give away and will not be passed to any other parties without your agreement.
